This template helps managers provide a comprehensive evaluation while incorporating self and peer feedback as optional sections. It keeps the focus on both accomplishments and areas for growth, supporting a balanced and structured review.
1. Review of Self-Assessment (Optional)
- How well does the employee's self-assessment align with your view of their performance?
- Comments: Provide feedback on their self-assessment, noting any areas of agreement or discrepancy.
2. Review of Peer/360 Feedback (Optional)
- How does the peer feedback support or differ from your observations?
- Comments: Highlight key insights from peer reviews, including any themes or specific examples that reinforce or contrast with your assessment.
