This template helps managers provide a comprehensive evaluation while incorporating self and peer feedback as optional sections. It keeps the focus on both accomplishments and areas for growth, supporting a balanced and structured review.

1. Review of Self-Assessment (Optional)

  • How well does the employee's self-assessment align with your view of their performance?
    • Comments: Provide feedback on their self-assessment, noting any areas of agreement or discrepancy.

2. Review of Peer/360 Feedback (Optional)

  • How does the peer feedback support or differ from your observations?
    • Comments: Highlight key insights from peer reviews, including any themes or specific examples that reinforce or contrast with your assessment.

3. Goal Achievement

  • To what extent did this employee achieve their goals for the review period?
    • Rating:
      • 1 - Did Not Meet
      • 2 - Partially Met
      • 3 - Met
      • 4 - Exceeded
      • 5 - Greatly Exceeded
    • Comments: Describe their progress toward goals, including specific accomplishments or challenges faced.

4. Quality of Work

  • How would you rate the quality and consistency of this employee’s work?
