Example performance evaluation form

This template allows direct reports to evaluate their manager in areas critical to team success and personal development, such as clarity, support, communication, and morale. It provides managers with actionable feedback on what’s working well and where they can better support their team members.

1. Support for Growth and Development

  • How well does your manager support your growth and development?
    • Rating:
      • 1 - Limited Support
      • 2 - Some Support
      • 3 - Good Support
      • 4 - Strong Support
      • 5 - Exceptional Support
    • Comments: Describe specific ways your manager has helped you grow in your role, such as offering feedback, providing learning resources, or mentoring.

2. Clarity of Vision and Direction

  • How clear is your manager’s vision and direction for the team or project?
    • Rating:
      • 1 - Lacks Clarity
      • 2 - Somewhat Clear
      • 3 - Clear
      • 4 - Very Clear
      • 5 - Extremely Clear and Inspiring
    • Comments: Include examples of how the manager communicates the team’s purpose, goals, or long-term vision, and how this impacts your motivation and focus.

3. Task and Role Clarity

  • How effective is your manager at providing clarity on tasks and responsibilities?
    • Rating:
      • 1 - Rarely Clear
      • 2 - Sometimes Clear

      • 4 - Mostly Clear
      • 5 - Always Crystal Clear
    • Comments: Describe how your manager communicates expectations for tasks and your role, including examples of helpful guidance or areas where clarity could improve.

4. Communication and Feedback

  • How would you rate your manager’s communication style and feedback?

    • Comments: Discuss how your manager’s communication style and feedback affect your work, mentioning any ways they make you feel heard, valued, or challenged constructively.

5. Process and Structure

  • How helpful are the processes and structures your manager has put in place?

  • How approachable and available is your manager for questions or support?

      • 5 - Extremely Approachable and Always Available
    • Comments: Provide examples of times your manager has been supportive or approachable, and any suggestions on how availability could improve.

7. Encouragement and Team Morale

  • How well does your manager encourage team morale and a positive work environment?

  • What are some areas where you feel your manager could improve?
    • Comments: Identify specific areas where you believe they could make changes that would positively impact the team or your work experience.

9. Additional Support or Resources

  • Are there any additional support, resources, or adjustments you need from your manager to succeed in your role?
    • Comments: Include any resources, feedback styles, or adjustments in approach that would be helpful for your work.
