This template allows colleagues to provide feedback on each other’s performance, strengths, and areas for improvement. It helps individuals gain a more holistic view of their work and identify opportunities for growth.

1. Goal Achievement

  • How well did this person meet their goals for this review period?

    • Comments: Describe any specific contributions or achievements they made toward their goals.
